Davidson Films, Inc : Fax Order Form

Use the form below to place an order by fax, using a credit card and/or a purchase order number issued from your organization. Print a copy of the blank form, then fill it out and fax to us. Be sure to print an extra copy of the page out for later use. We will confirm the order by e-mail within 48 hours. We ship via UPS. The shipping charge for the first video is $11.00 and we charge $1.50 for each other video in the order. There is no handling charge. Your credit card will not be charged until we ship the order. We offer a 15% discount on orders of two or more films, and we also have special prices on selected film packages. Your emailed order confirmation will contain the actual order cost, with the calculated discount, if applicable, and the shipping charges detailed with the total. Our sale terms for purchase orders are Net 30 days from receipt of the shipped order from Davidson Films. Please do not send us a duplicate purchase order. This form is all we require.
